CMC Initiative grants you the chance to privilege from 70% discount, although Certain patients will be eligible for 100% discount, for 8 week-period of treatment with either CollaMouv® or CollaMouv Plus®. CMC aimes to help patients to alleviate the burden of Osteoarthritis and GO back with their lives again.
How to get the most of CollaMouv® Care (CMC) Program?
1. Be a CollaMouver by creating your account on our website.
2. Ask your Physician or Pharmacist to fill out the below (FACIN) form
3. Your Physician or Pharmacist will provide a list of candidate patients to take part of CMC Support Program
4. Make sure to give your CollaMouver Account email to your Health Care Professional while he's in communication with us.
5. Once your Health Care Professional give us your CollaMouver account, we will be in contact with you either by Email or Telephone to inform you how to get your CollaMouv® or CollaMouv Plus® Drinkable Shots for 8 Weeks.
